Gaming Reporting Forms
This form is what needs to be filled out after EACH 50/50 draw and EACH raffle table draw. A new, blank form is to be used for each draw. These are filled out and saved until the end of the year by the team manager, or in the case of a tournament raffle given to the SOMHA tournament coordinator at the conclusion of the tournament. (this is an official GPEB form)

We have completed a sample form for a fictitious 50/50 draw and have provided annotations on how to fill out the form for 50/50 draws and tournament raffle draws.
Gaming Event Revenue Report
This is another GPEB form. This one is used to CLOSE your Class D license. Therefore it is used at the end of the tournament to close the raffle ticket license and also used at the end of the year to close the team’s Class D license for 50/50. The information collected on the Gaming Reporting Form is used to fill this form out. It is to be given to the SOMHA tournament coordinator at the end of the tournament, or in the case of a team’s 50/50 license, given to your division Director at year’s end. We will ensure accuracy before sending them to the Province.

Class D Guidelines
Each team is required to apply and produce a Class D gaming license for 50/50 sales at home games. Each tournament is required to apply and produce a Class D gaming license for raffle ticket table draws.
